How Do I Get Started?
Clicking the button below will open a new browser window for our My Secure Practice forms for new patients. Using this system, you can easily fill out the form right on your screen!
When you're done, simply click the 'Complete & Send' button at the bottom, and your information will be sent directly to us, using encrypted technology to ensure everything is kept private and completely confidential.
As mentioned above, if you prefer to print the form, click the download icon in the upper righthand corner, print the form, and use a pen to fill out each section. Please be sure to then bring the completed form with you to your first appointment.
